About the position

Responsibilities

  • Serve as a Care Coordinator for individuals receiving support through Home and community-based Medicaid waivers or ICF/IID.
  • Utilize a person-centered planning process to design support plans for recipients with diverse needs.
  • Make regular in-person contacts with all HCB waiver recipients assigned and maintain contact with applicants, families, care providers, and stakeholders.
  • Provide consultation to ensure compliance with service principles, program standards, regulations, and statutes.
  • Represent professional decisions throughout the Fair Hearing appeal process as necessary.
  • Gather, maintain, and assist with analyzing, interpreting, and reporting programmatic data to improve service efficiency and quality.
  • Provide technical assistance and conduct home and/or site visits as directed.

Requirements

  • Must meet the definition of a Qualified Intellectual Disabilities Professional (QIDP).
  • At least one year of experience working directly with individuals with intellectual or other developmental disabilities.
  • A Bachelor's degree from an accredited college in biological, health or behavioral science, health practice, education, public, healthcare, or business administration, or a closely related field.
  • Progressively responsible professional experience in health program planning, development, coordination, evaluation, or implementation.
